Initial Assessment: $100.00
Follow-up Treatment: $90.00
Student Initial Assessment: $90.00
Student Follow-up Treatment: $80.00

Direct Billing is currently available with Medavie Blue Cross, as well as most other insurance companies through Telus e-health (Manulife, SunLife, Great West Life, Desjardins and Chambers of Commerce).

Payment accepted by Interac (Debit), Visa, Amex and Mastercard. E-Transfers are are also accepted. Cash or cheques not accepted.

Please note the clinic has a 12 hour cancellation policy. Any missed or appointments cancelled within 12 hours of the clinic visit, will be charged 50% of the cost of the appointment. Our full cancellation policy can be found here.

If you have experienced extenuating circumstances preventing required notice to cancel an appointment, please outline your reasons in our No-Show Waiver Request Form. 

If this is your first time attending the clinic, we have 3 forms you will complete at your first appointment. If you prefer, you can download the forms below and complete them electronically, or print them, fill in by hand and bring them with you to your appointment.

Electronic completed forms: Please save with your name (Last name, First name) at the beginning of the file name, then e-mail to 'ptclinic@dal.ca' with your name in the subject line.

Access to Dalplex for your appointment:

Please note that the Dalplex client experience centre will require proof of your physiotherapy appointment upon your sign in for safety and security reasons, if you do not have a Dal Card or a Dalplex Membership card. 

To gain entry without a card, please show your appointment confirmation email or SMS message to the Dalplex staff when signing in. If you forget your device to gain entry, please give your verbal consent to the Dalplex staff to call the DPC to gain entry by appointment confirmation.

If you have booked an appointment same day, you will be required to give verbal consent to the Dalplex staff to call the DPC to gain entry by appointment confirmation. 

Note: This process does not apply to entry of our location inside the Oulton-Stanish Centre.
